If you plan to join a hosted group dinner Wednesday evening, please list the number of attendees by your choice (everyone will pay for their own meal):





The conference fee is $200


Guests Attending:


 

The guest fee is $70 which includes three meals and social activities (Please select the correct number for quests, after adding to Pay-Pal cart.)


This form must be accompanied by a payment of $200 per person, plus any additional guest fees. You will not be considered registered until payment is received.
